The Index was started originally in the 1930's by Professor Robert P. Blake of Harvard University as a cumulative index of bibliographical entries in the Byzantinische Zeitschrift. In 1942 the enterprise was transferred to Dumbarton Oaks in Washington, D.C. Conceived primarily as an alphabetical author catalogue, the index was enriched further with references cited in Krumbacher. Concurrently, a catalogue of Slavic authors in the Cyrillic alphabet was completed with references from 77 journals published in Russian and other Slavic languages, most of them before 1917.

At present, the Author Index II occupies at Dumbarton Oaks 94 drawers with over 100,000 cards in the catalogue cabinets. The arrangement of the Index is alphabetical by authors – classical, mediaeval, and modern - in one sequence. Under the classical authors both their texts and modern studies about them are included. In addition to the author file there are the following sections: Titles, representing mediaeval anonymous works as well as modern collective works; Festschriften, in which all articles published in a Festschrift are collected under the name of the scholar in whose honor the volume was issued; Personalia, where information on scholars is included; Catalogues, section in which are collected museums and exhibitions catalogues, those of the private collections, and, sales catalogues. The last section is on Congresses, where information on articles published in the Acts and Proceedings of various congresses are recorded.
