Acknowledgements

This book grew out of my dissertation and is therefore the final step in a research and writing project that has spanned several years. The book would not have been possible without the support I received as a doctoral student at the University of Victoria. My upmost gratitude goes to my doctoral supervisor, Bill Carroll, who encouraged this project from its earliest incarnation and provided invaluable intellectual input and guidance throughout the entire process.
