Since this book is a revised version of my PhD dissertation, I am indebted to my supervisor, Zoltán Szombathy, for his invaluable guidance throughout my doctoral studies. My deep appreciation goes out to the reviewers of my dissertation, Anne Regourd and Elaine van Dalen, as the process of revision greatly benefited from their suggestions.
